Report Drug Diversion

Reporting suspected drug diversion is critical to stopping it. Anyone can report or volunteer to add incidents to our database and map.

Report Drug Diversion

Thank you for joining our community against drug diversion. Reporting is an impactful first step. Please fill out the following form with as much information as possible.

Visit our Resource Center to learn more:

  • What is considered drug diversion?
  • Report diversion to law enforcement and state boards.

  • Establish drug diversion best practices in your hospital.

Anyone can submit. You may submit anonymously
We will accept submission of all types of incidents: criminal cases, licensure cases, incidents of suspected diversion, etc.
We forward some reported diversion incidents to other agencies (see FAQs for more details)
The more detail, the better, especially for trending/mapping
We only publish what we can verify, so please be thorough
We will “de-identify” incidents where diversion has been suspected but not indicted or proven in a court of law or licensure board

Edit or Refute

Rebuttal and retraction requests will be accepted and reviewed for possible incident editing and/or deletion. Please submit a request here.

Healthcare Diversion Network will revise a posted incident if new information is submitted about the incident. For example, if and when a person is convicted of diversion, the incident posting will be revised to show their name for more transparency. Healthcare Diversion Network will delete a posted incident if a court of law or a state licensure board requests it be deleted.

Healthcare Diversion Network reserves the right to revise or delete any incident that has been posted on its website.
